Where can I find the Food Dealers Permit online for establishments?

My school is requiring that we submit the Food Dealers Permit from the place where we're buying food from to sell. I've looked online but so far I've only found the site that tells you about the food violations for establishment. Does anyone know where I can find the online version of the Food Dealers Permit for establishments?
