Seeking Affordable Care Facility

My grandfather, whom has dementia, has had some additional health issues that have left him bed-bound. Things have progressed to a point that have pushed us to seek a care facility for him. I've made some calls, but local options are looking more expensive than I'd initially realized. Can anyone recommend affordable facilities within a 4hr drive of Houston?

Considering pricing can only be provided by calling each home, I'm specifically hoping you can provide places you know to be more economic and potentially the pricing for those places. Really hoping for options less than $5k/month and willing to drive a bit (~4hrs) to find care.
