Mail-in ballots – how to apply?

Hi all,

Please keep politics out of this post.

Interested in signing up for mail-in voting. I pulled the app from https://www.harrisvotes.com/Docs/VotingInfo/Ballot%20By%20Mail%20Application%20-%20English.pdf and i don't meet the criteria (65+ years old, disability, absent from county, in jail).

Is there another way to apply or am i stuck with going to a polling place?
