Loved one is dying and I need advice

I have a close relative that has very little savings and only gets a deceased spouse’s social security check. They don’t have any health insurance and don’t want any help. They have no will and no medical directives. They never went to the doctor in the early stages because they just knew it would bankrupt them and Now they don’t want to got to the doctor because they are accepting their fate. I feel they need hospice care at the very least, but without insurance I’m not sure how to go about it. They are 63 and don’t qualify for Medicare. I really need some advice for our next steps. We are in the Spring area. This is falling on my souse and I and we aren’t sure where to even begin.
