Looking for a designer/tailor for wedding dress redesign! HELP!

I am getting married in December and want to have my mom’s dress altered/redesigned for me. Has anyone worked with an amazing dress maker or tailor who redesigned an heirloom dress for you? Any recommendations welcome within 5hrs drive!
