LGBT and "DIY" friendly primary care doctors?

Suffice it to say I'm not connecting well with my primary care physician at the moment and I am looking to try to connect with someone who is more my speed.

Unfortunately I'm what's known in the medical community as a "pain in the ass" in that I generally know what outcome I would like and am fairly comfortable doing the legwork to see if a medication might be a good fit for me.

The big thing I've been dealing with lately is a bunch of stuff with libido and gender identity… so I am lowkey looking for an LGBT friendly doctor who wouldn't mind if their patient wanted to discuss hormone therapy from either side of the equation. lol.
