ISO Home Health Care Recommendations

I'm looking for a Home Health Care agency that can help with my father.

Our family moved him into an independent sr. apartment inside the loop (near Main and Kirby). But he needs help going to doctors and understanding what they're telling him. I don't live in Houston.

We hired a woman who does a good job (she sits in on Dr visits, provides transportation (sometimes), reminds him what to do, keeps a log, helps him manage his medication). But his long term care insurance is looking for someone calling themselves "Home Health Care" and she calls herself "Care Management". That may sounds silly, but like I said, insurance.

So we're looking for someone who can provide these services but call themselves Home Health Care so insurance will pay. Any recommendations?
