Is this a health insurance scam?

Hello. I have Community Health marketplace insurance. I recently received the "Texas Department of Insurance Required Disclosure Statement" in the mail, asking me to sign it to make sure I agree to the fact that 2022 my plan has a deductible. It included a prepaid envelope to the address "PO Box 2963, Houston TX."

However, this is not the address listed on their website, nor is it the address that the invoices are paid to. Googling it seems to return nothing. So I thought it might be some scammer posing as their company.

I submitted an email message on their website to ask if PO Box 2963 is their address or not, but the representative asked me to scan the form itself and send it to them. Seems they don't understand what I'm saying. So I called them and another representative said he has no idea what that form is and can't help at all. (what kind of customer service is that??)

All I'm trying to find out is a yes/no answer to whether "PO Box 2963" is actually their address or not.

Does anyone here know anything about this? Seems so bizarre that this company's employees can't tell you the company's address. I can't tell if this is a scam or if the company itself is so clueless that they just have no idea.
