Houston Health Department Survey

I received a survey from the Houston Health Department about " thoughts related to COVID-19 vaccines". I cannot get past page 1 of the survey because the city I live in and consequently my zip code is not included. I live in Harris County. I'm confused but not shocked it's not included. I guess the ~191k residents that live here don't count.
