Hip Hop / Swishshouse / Screwston – what does “my fifth wheel just died” mean

Ok I’ve been listening to htown Swishahouse music for a decade now and I’ve heard a few times “my fifth wheel just died” but I don’t understand this reference. Google isn’t much help. Is this a local thing or am I just a dunce?
