Galveston Island elderly transport service, or doctor house call.

My mother lives in Galveston and is very rapidly going down hill, to the point where she cannot walk. I'm trying to set up a doctors appointment this week, but I don't know how to get her there. Is there a transport service that anyone could recommend?

I'm also open if anyone could recommend a doctor that would do an in home visit.

She doesn't have a doctor currently, she's only lived there 5 years and just hasn't needed one up until now.
