Can somebody explain me Harris Health System plans?

I currently got Plan 4. I don't know why I got Plan 4 even though I don't make much. I had to pay $95 per visit for plan 4. I don't know where I can look up information for this. According to my pay stub, I make just over 30k per year. What is the requirements for other plans?
