Hey all,

I require a SMART Health Card for upcoming travel, which can only be issued by the vaccine provider or government entity. Many countries now require this as the only acceptable form of proof of vaccine status. I was vaccinated at a city of Houston site via UMMC.

I have spent considerable time on the phone with UMMC (the provider the city uses), the city of Houston Health Department, the COVID hotline, the county, and the state immunization unit at the state health department, all leading to essentially no one who has any idea how to provide this.

Given the number of people vaccinated by the city and the growing standard of the SMART Health Card, I’m assuming someone else is in this position and might have some advice on how to get a solution to this?
