Any vet recommendations for a dog with ACL tear?

My almost 12 year old dog can't get a break, he just tore his left ACL. About four years ago he tore the other one (rigth side) we never had surgery on it due to health issues on advice of vet, and he seeemed to be doing good. Now that he tore the other one, I'm thinking he may need surgery, if its even possible now, on both of them. My vet doesn't do the surgery. Anyone had this done in Houston and had good experience? Anyone had a similar situation where one tore and didn't do surgery, and then the other tore? I'm devastated, this dog is my world, but I don't want him in pain. I'm not even sure if surgery will be an option, he is diabetic, has high liver and pancreas labs, and has IBD that is so hard to control. Im thinking at this point the risk of surgery might outweigh the alternative. Any advice or vet recommendations???
