After stroke care for uninsured

My family is dealing with a relative who had a stroke and had to be hospitalized, have surgery and is basically now unable to care for himself. Before the stroke he was unemployed, and uninsured. He has no money. He is currently at a hospital in Montgomery county but we’re trying to find him a place to go to for after stroke care- in patient rehab center or something like that. The social worker at the hospital said it’s pretty impossible to find a charity bed somewhere. They said he’s almost ready to be discharged. He is 51 and I guess won’t qualify for Medicare. The cost of in home care won’t work.. Are there more options for him in Harris county? We’re at a loss on what to do…anyone ever run into this situation before?
