Hospice Services in the Area

Hello. I have a family member in the Houston area (Polk County) that needs Hospice services. I am 1,800 miles away, though.

Are there any healthcare workers here that could chime in about end-of-life, Hospice options in the area?

I’m hoping to help set this family member up with Hospice at home or a facility so they can live these last few days pain free.
