Recommendations for LTACH (long term acute care hospital)?

Had anyone had a good experience with any LTACHs in the Houston area? My grandma is currently in ICU and will likely need to go to a long term facility with intubation and a ventilator. We had a terrible experience with Kindred Sugarland, and I can’t seem to find a facility with good reviews.

If she comes off the ventilator successfully she would need a skilled nursing facility instead, so if you have any recommendations, that would be really helpful too.
