Unemployment question

Im trying to apply for unemployment for my mom and i cant seem to get past a certain page. https://imgur.com/a/cc9WIfH this is about as far as i can get and every single time it tells me just to call them. when i applied for myself i had zero issues but i cant seem to get it done for my mom. I can get to the page where it asks for her driver license number and stuff like her bday and first day she worked and last day she works. it then takes me to the top part of the picture and the second i hit Next it takes me to the bottom part of the picture. Can anyone help me out?
