GP/PCP recommendation near 290/610 area?

I haven't had a regular, consistent doctor for a number of years now, and I'm looking to find a good doc that I can trust and rely on so I can stop going to Urgent Care if I need to see someone, preferably on the NW side close to the 290&610 area.
